DBE Program -- Regulations and Requirements

General Requirements

Participation on the Music City Center construction project requires all subcontractors, vendors, and suppliers to be pre-qualified in order to bid on and receive contracts. Below is a checklist of information required to complete the qualification application:

Checklist for Subcontractors, Vendors, and Suppliers

Company Information

  • Company name, mailing address, phone numbers, and e-mail
  • Federal  ID and Dun & Bradstreet number (if applicable)
  • Business type, year founded, and state in which it is incorporated, if applicable
  • Officers of the company and length of service
  • Trades performed by company
  • Business classification: certifying agency, certification number, expiration date

Licenses

  • States in which company is licensed and license numbers, trades in which company is licensed

Work Experience

  • Number of employees
  • Number of contracts completed in last five years
  • Largest contract completed by company, including name of project, dollar amount of contract, and scope of work
  • Largest contract your company expects to complete this year, including name of project, dollar amount of your contract, and scope of work
  • List your top five current projects, with name, location, owner, architect and engineer, and prime contractor

  • Has your surety ever finished one or more of your construction projects? (Yes/No)
  • Has your company ever petitioned for bankruptcy, defaulted, or had a contract terminated? (Yes/No)
  • Has your company ever been barred from doing public work? (Yes/No)
  • Has any entity ever made a claim in a court of law against your company for defective, improper, or nonconforming work, or for failing to comply with warranty obligations? (Yes/No)
  • Has any entity made a claim in a court of law for failing to make payments? (Yes/No)

Financial Information

  • Revenue and projects for previous year and projected for current year
  • Backlog: current and 12 months ago
  • Current net worth and net working capitol
  • Bank credit line, including credit limit and percent in use
  • Bonding capacity, both aggregate and per project
  • Profitability for last five years

References (Provide company names, contact names, and phone numbers)

  • Major suppliers (3)
  • Prime contractors (3)
  • Banks (3)
  • Surety (not insurance broker or agent)

Safety

  • Safety Coordinator – Provide name, title, phone, and qualifications or experience
  • Substance abuse screening – must screen pre-hire, for cause, post-accident, and random
  • Safety training – Must provide training upon hiring, for supervisors and managers, and offer plan for “tool box” meetings
  • Provide list of OSHA recordable incidents, OSHA violations and fatalities for the last five years
